• A Dean's letter from medical school.

• Three letters of recommendation. One must come from the internal medicine residency program director.
• A curriculum vitae and personal statement.
• A valid ECFMG certificate if graduated from a non-U.S. or Canadian medical school.

It is crucial for applicants to possess a valid permanent visa or U.S. citizenship. The program specifically does not accept H1B visas. Moreover, candidates must hold a permanent Illinois medical license upon starting the fellowship. Only completed applications will be reviewed.
