Overview

Overview of the Critical Care Medicine Fellowship Program

The MaineHealth Maine Medical Center Critical Care Medicine Fellowship is a distinguished ACGME-accredited program designed for advanced training in all aspects of critical care, with a focus on state-of-the-art clinical practice and research methodologies. This Internal Medicine-based fellowship is open to candidates who have completed prior training in internal medicine, med-peds, or emergency medicine. It consists of a rigorous two-year curriculum, although one-year options may be available for those completing specific internal medicine-based subspecialty fellowships such as cardiology, pulmonary, infectious disease, or nephrology 1.

Curriculum

The fellowship structure encompasses an extensive two-year curriculum comprising various clinical experiences and educational activities, broken down into:

  • 8 months in medical intensive care
  • 2 months in surgical intensive care
  • 2 months in neurocritical care
  • 2 months in cardiovascular critical care
  • 1 month in anesthesia
  • 9 months in electives/research

Fellows are encouraged to engage in several weekly conferences that include core curriculum series, internal medicine grand rounds, and critical care research seminars. There are various additional monthly educational activities aimed at consolidating clinical knowledge and improving skills in diagnostic and procedural aspects of critical care medicine. Examples are simulation and journal clubs, morbidity and mortality conferences, and monthly critical care grand rounds 3.

Rotations and Unique Experiences

The fellowship provides crucial clinical exposure in multiple specialty areas:

Medical Critical Care

The focus is on developing comprehensive medical decision-making skills necessary for the supervision of critically ill patients, which involves mastering a variety of clinical procedures and understanding complex end-of-life care issues 4.

Neurocritical Care

As Maine's primary referral center for neurological conditions requiring ICU care, fellows gain substantial experience treating stroke patients, managing intracerebral hemorrhages, and dealing with neurocritical emergencies in collaboration with neurology and neurosurgery specialists 4.

Surgical Critical Care

Fellows develop competencies in managing critically ill surgical and trauma patients while gaining insight into pertinent critical care procedures 4.

Cardiovascular Critical Care

This rotation encompasses experience in both cardiac and cardiac surgery intensive care, allowing fellows to engage in evaluating and managing acute coronary syndromes and related procedures 4.

Research and Academic Opportunities

The fellowship emphasizes the importance of research and scholarly activity. Fellows are encouraged to conduct research projects and present findings at local, regional, or national conferences, as well as publish in peer-reviewed journals. This aspect is critical for establishing the fellow's presence and leadership in the field of critical care medicine 5.

Faculty and Mentorship

The program features a dedicated team of board-certified critical care specialists from various medical backgrounds. Faculty members are committed to providing high-quality teaching experiences and mentorship, ensuring fellows are well-prepared for independent practice in critical care and achieve personal and professional growth 6.

Diversity, Equity, and Inclusion

MaineHealth Maine Medical Center emphasizes fostering a welcoming and respectful environment for all fellows. This diversity initiative promotes collaboration, empathy, and mutual respect across different backgrounds and experiences, which enhances learning and patient care outcomes 1.

Application Process

Applications to the Critical Care Medicine Fellowship are accepted through the ERAS (Electronic Residency Application Service). Required documents include a letter from the residency program director, two additional letters of reference from U.S. teaching hospital attendings, and transcripts of USMLE or COMLEX scores. Notably, the program currently accepts J-1 visas only for international candidates.

Interviews are by invitation only, and only complete applications will be reviewed. Selected candidates will be notified via email or traditional mail regarding their interview status 2.
