The Critical Care Medicine Fellowship at the Medical College of Wisconsin is a two-year ACGME-accredited program focused on developing competencies in the clinical and basic aspects of critical care medicine. This fellowship is tailored for graduates of internal medicine residency programs who aspire to excel in critical care settings.

Program Structure

The fellowship consists of two primary training sites. The first is Froedtert Hospital, a large, renowned tertiary care center with 536 beds, frequently ranked among the top academic medical centers. The second site, the Zablocki VA Medical Center, is recognized as one of the premier VA clinical sites in the U.S. Both institutions provide a rich and diverse setting for clinical training.

Central to the program is the involvement in closed medical intensive care units (MICUs). Froedtert's MICU comprises 26 beds, while the VA MICU includes 18 beds, where fellows take on responsibilities akin to junior attending roles. The fellowship emphasizes specialized rotations, including surgical/trauma critical care, neurointensive care, cardiovascular ICU, and transplant ICU.

Didactic and Clinical Training

Fellows will complete core didactic sessions in their early training period, focusing on essential topics such as common disorders in critical care, ventilator management, hemodynamics, and echocardiography. Another integral part of the curriculum involves the advanced critical care physiology series, which delves into complex physiological principles essential for managing critically ill patients.

Additionally, a structured Friday conference provides protected time for educational lectures and board review, featuring both faculty and guest lecturers to broaden fellows' expertise. Monthly morbidity and mortality conferences are also held to enhance learning from real clinical cases.

Research Opportunities

Research is heavily encouraged throughout the fellowship. Fellows are expected to engage in projects within Pulmonary, Critical Care & Sleep Medicine or related areas. They benefit from dedicated research blocks during their second and third years and have access to well-equipped laboratories and experienced research coordinators who assist with protocol development and IRB submissions. The program has a strong track record of fellows presenting at national conferences and publishing in peer-reviewed journals.

Procedural Training

Fellows gain extensive procedural experience, including endotracheal intubation, central venous access, and bronchoscopy. They perform these procedures under direct supervision, ensuring they achieve full competency. Simulation training is also provided to enhance their skills, especially for critical procedures like ECMO management and ultrasound techniques.

With opportunities available for experience in various procedures such as lung ultrasound and tracheostomy management, fellows are well-prepared to handle the demands of critical care practices.
