Transitional Year Residency Program

The Transitional Year residency program at Tripler Army Medical Center is designed to provide medical graduates with a well-rounded foundation in various clinical disciplines. It emphasizes both the preparation for subsequent specialty training and the capability to serve as a General Medical Officer. This one-year program is accredited by the Accreditation Council for Graduate Medical Education (ACGME) and accepts 17 residents per year.

The mission of the program is to ensure safe and effective patient care while equipping residents with the skills, knowledge, and attitudes necessary for their future medical careers. The program aims to develop respectful, professional, and competent first-year physicians capable of managing a wide range of illnesses. Additionally, it promotes ongoing professional growth and provides a supportive learning environment.

Curriculum and Rotations

The curriculum consists of 13 four-week block rotations tailored to resident interests, with possibilities including General Surgery, Pediatrics, Obstetrics/Gynecology, Emergency Medicine, Intensive Care Medicine, and others. Residents can choose three elective rotations from various specialties, such as Allergy & Immunology, Cardiology, Psychiatry, and more.

During these rotations, residents will participate in numerous didactic activities, including morning reports, lectures, and journal clubs. Monthly Transitional Year didactic meetings cover essential topics such as military medicine, patient safety, and medical ethics. Residents also receive training in Advanced Cardiac Life Support, Advanced Trauma Life Support, and Combat Casualty Care.

Scholarly and Professional Development

The program does not have a formal research requirement; however, residents may establish a research rotation to engage in Quality Improvement projects. Each resident is expected to present a quality improvement overview halfway through the academic year.

Professional development opportunities are robust, as residents can teach medical students from other clinical rotations and fellow TY residents during monthly meetings.

Training Sites

The primary training site for the program is the Tripler Army Medical Center, with additional rotations at the Queens Medical Center, both situated on Oahu, Hawaii. This enables residents to work at a facility that services a broad population, including active-duty military personnel, family members, and veterans.

Application and Competitiveness

The applicant profile is highly competitive. The program does not specify mandatory interview requirements for applicants but emphasizes the need for USMLE Step 3 or COMLEX Level 3 completion during the training year. It is recommended that candidates complete this examination by March of the academic year.

Visa Support

The program offers visa sponsorship for eligible international medical graduates, providing support typically for J-1 and H-1B visa applications. This is crucial for IMG candidates looking to pursue residency training within a military medical setting.
